2009 Registration
A team shirt, cap and socks are provided with registration fee.
Please bring one proof of age document such as a birth certificate or other original document issued by state, federal or registrar of vital statistics. A copy cannot be accepted. Please bring 3 or more documents that show the address of the legal residence of the parent(s) or court appointed guardian(s) for the player(s). These can include driver's license, electric or other utility bill, and a financial record like a credit card statement or loan statement. PLEASE NOTE: three utility bills count as only one proof. Also bring payment for the registration fees.
