Player Registration form, Medical Release form and a copy of the players birth certificate MUST be completed and delivered to the league before your player can be placed on a team. Please print the medical release form and provide it along with your child's birth certificate and proof of address to League via email attachements (preferred), in person to walk-in registration, or to a League official.
