Policy & Registration Forms

All new members must acknowledge receipt of program policies and complete the necessary registration forms in order to participate in any CheerWorkShop program. All returning members must update registration information once per year in June.

The Absence Request Form is to be completed and submitted for approval one (1) week prior to a cheer member's absence.

IMPACT Program Info & Forms

The IMPACT (Individual Member Payment Assistance for Cheerleading & Tumbling) program was created to ensure that all youth, regardless of socioeconomic status, would have the opportunity to participate in a quality competition cheerleading and/or dance program without it being a financial burden for the family. At the present time, ONLY competition cheer and/or dance members are eligible to apply for financial assistance through the IMPACT program. Please read over the information to determine whether or not you qualify to receive this assistance!
