The Chiefs are accepting coach's applications for the 2010 season. All applications will be considered by our selection committee. We welcome applications from our current coaches and new volunteers who can maintain and help acheive our high standards.

Coach's Application

Valley Youth Conference player contract and registration form. Fill out this form, print it and bring it to the next registration. Please include your Email address in a prominent place on the contract, and please print it clearly.

VYC Contract

This form MUST be completed, signed and stamped by a Doctor and returned to the Team Manager BEFORE THE ATHLETE WILL BE ALLOWED TO PARTICIPATE IN ANY ATHLETIC ACTIVITY - get this done before August 1st!

We will have a Doctor present to perform physicals for a $15.00 fee at our picnic on July 25th @ Chatsworth H.S.

Physical Form

Every player, parent, coach and anyone connected with a team must sign this document. Print it out, fill it out and give it to the manager of your team. Or bring it to registration.

Code of Conduct

The Chiefs are always looking forward to our parents volunteering their time to help make the season go smoothly and pleasantly for the kids (and us).
Please print this form, indicate where you feel you can make the greatest contribution and give the completed form to your manager.
