Player Forms

REGISTRATION: Player Contract, registration fee and a copy of players' Birth Certificate are required. An Age Waiver is required if player "plays up" in an older division (i.e., a 1998 birthdate playing in the 12U division)

The Authorization for Medical Treatment form is notorized and turned in to your team Manager.
