ON-LINE REGISTRATION
Please note the on-line registration is due on November 30, 2011

HARD COPY REGISTRATION FORMS due January 15, 2012
In addition to the on-line registration the following hard copy registration forms must be completed and returned

2) Central Bucks School District - Emergency Information and Consent

3) Student Pledge (found in the Student Athlete Handbook)

4a) PIAA Comprehensive Initial Pre-Participation Physical Evaluation CIPPE
(sections 1 - 4) Note: to be completed by Authorized Medical Examiner
                  OR
4b) PIAA Comprehensive Initial Pre-Participation Physical Evaluation CIPPE
(section 5 Re-Certification by Parent/Guardian)

NOTE: If a student participates in multiple sports during the same school year, the student-athlete’s parents will need to complete section 5 (re-certification portion) of the CIPPE prior to the start of the new season
