I understand that by signing this document I am committed to the above team and to the league, which is a summer baseball league. I have not signed, and will not sign, any other commitment form for the 2010 season. Signing this document prohibits me from participating on any other sanctioned summer league team. I may participate on other outside teams only according to team and league regulations.

I understand that this is a direct offer of the team to fill a place on its team roster in accordance with these rules of the league.

I understand that I may be released from this letter of commitment only in the event of my signing a professional contract; being selected to an active traveling all-star team, the US Olympic Committee or any of its subordinate Committees, or a similar amateur athletics authority, or by written mutual consent of the commissioner of both involved leagues.

I understand that this letter of commitment is not valid until each of the requested signatures is obtained.

CAROLINA VIRGINIA SUMMER COLLEGIATE LEAGUE AUTHORIZATION AND RELEASE STATEMENT


Catawba Valley Stars is organizing a baseball team to compete as a member of the Carolina Virginia Summer Collegiate League.

I/We do hereby, fully and forever release, discharge and hold harmless Carolina Virginia Summer Collegiate League, its Agents and employees from any and all claims for property damage, personal injury or death that may arise from my/our Child’s participation with the Carolina Virginia Summer Collegiate League, including, but not limited to, any and all claims arising from the negligence, negligent supervision, or carelessness of Carolina Virginia Summer Collegiate League. 

I/We fully understand that playing baseball can be extremely demanding, and poses the risk of contact, either with other players, baseballs traveling at high speeds, as well as other types of personal injuries that may result from participation in practices and/or games, and that my/our Child’s participation in this activity may lead to permanent physical restriction imposed by a physician, and that I am unaware of any health related problem that may affect my/our Child’s ability to participate in sporting events.

By executing this document, I/we certify that my/our Child’s participation on this team, and my/our signature on this document, is completely voluntary, and that I/we are acting of my/our own free will.

I/We hereby agree that this document shall be governed and construed under the laws of the State of North Carolina. I/we also understand that I/we are free to ask the advice of independent legal counsel, at my/our own expense, prior to executing this document; and that any advice, suggestions or statements made by any agents or employees of Carolina Virginia Summer Collegiate League regarding legal issues has no bearing on my/our execution of this document.

I/We also agree that the Carolina Virginia Summer Collegiate League, and any vehicles they may operate, are not common carriers, and as such, owe no greater standard of care than any other party.

Text Box: TEAM POLICIES

1)	BE ON TIME – Report to all baseball activities on time, i.e. practices, team meetings, departure times.
2)	Each player is responsible for receiving approval from the Coach to report late or be excused from practice. Do not send word with another player.
3)	It is each player’s responsibility to keep his uniform clean and shoes shined.
4)	Uniforms will be worn on the field only.
5)	Curfew hours on all overnight trips will be announced and enforced.
6)	Drinking alcoholic beverages, smoking, or chewing tobacco is not permitted.
7)	The possession or use of illegal drugs will result in automatic dismissal from the team.
8)	Refrain from swearing both on and off the field.
9)	Do not throw any equipment, including bats, helmets, gloves, etc. at anytime.
10)	Arguing with umpires is not permitted. Umpires are to be addressed as “Mr. Umpire,” not “Hey Blue” or “Hey Ump.”
11)	NO cliques, no complaining, no criticizing no envying, no excuses. WE ARE A TEAM!!! – pull for one another and keep it positive!!!
12)	 Players in apartments are responsible for refundable deposit. Deposit is refundable if apartments are left clean and undamaged.




DISCIPLINARY ACTION

Disciplinary action for not adhering to team policies will be either a warning, suspension for a game, or games, or dismissal from the team.

I have read and understand the above rules. I agree to abide by and follow all rules. I fully understand the disciplinary action that may be taken against me in the event it becomes necessary.
