ORIGINAL DOCUMENTS
Certified True Copy Of U.S. Birth Certificate From A City Or Other Local Municipality, County Or State
Passport Or Immigration Visa • All U.S. Government Birth Certificates • Alien Registration Card
Adoption Papers, Wherein The Player's Name & Date Of Birth Is Properly Recorded

PHYSICAL EXAMINATION REQUIRMENTS
No Player
Is Permitted To Enter Into ANY Type Of Physical Training Without First Obtaining Health Certification
Our Chapter Will Schedule A Physical Examination
Sometime After The Last Registration & PRIOR To The FIRST Day Of Practice
The Fee For This Is NOT Included In Registration Fees & Is Set By The Medical Staff Selected
Physical Exam Fees Usually Cost About $15 Or $20

THE CARSON COLTS FOOTBALL SEASON
Beginning In The Month Of August
We Will Have Practices Every Week Day (Except Sundays) Until Players Have Obtained
Ten (10) Hours Of Conditioning
In September
Practices Are Reduced After Game #1 To Three (3) Days Per Week (Usually Tuesday, Thursday & Friday)
At Victoria Regional Park Located In The City Of Carson
Each Saturday
We Play Other Teams Within Our Region From September Until Late November At Various
Locations & Some Teams May Have To Travel To Play AWAY Games As Per The OCJAAF Schedule
Playoffs
The Last Three(3) Games Held In The Month Of November Are PLAYOFF Games
Which Determines The Teams That Qualify As League Champions
And Finally...
It's On To The SUPER BOWL Where The Winner Is Crowned The
ORANGE COUNTY JR. ALL-AMERICAN FOOTBALL CONFERENCE CHAMPIONS
For Their Respective Divisions

INSURANCE INFORMATION
Orange County Jr. ALL-American Football, Inc.
Provides Insurance By Gagliard Insurance Services, Inc. For ALL Participants In Our Program, Which Is Paid Through Chapter Team Fees.
This Insurance Covers All Coaches, Players/Cheerleaders, Team Management, Board Members, Or Anyone Else Directly Involved With Chapters Of OCJAAF. Further, This Insurance Also Covers Practices, Games And Any Fundraiser Events That Are Sanctioned By OCJAAF Policies.
Orange County Jr. All-American Football Is The Major Policyholder Of This Insurance. If You Have Any Type Of Medical Coverage, Those Policies Will Act As The PRIMARY Coverage Carrier.
Any Claims To Be Processed By OCJAAF Must Be Filed Within 90 Days From The Date Of Injury. Additionally, OCJAAF's Policy Requires A $250 Deductible For Any And All Claims Processed.
Any Deductible(s) Or Co-Pay Balances Are The Parent's Responsibility.
It Is Very Important For Everyone To Understand That In The Event Of An Injury Requiring Medical Treatment, All Parents Are Primarily Responsible For ALL MEDICAL BILLS... NOT THE INSURANCE COMPANY.
