EQUIPMENT RENTAL AGREEMENT

In consideration of borrowing equipment from the Capital Land Lacrosse & Field Hockey Club, Inc., the undersigned agrees to pay a non-refundable rental fee as well as to meet all other conditions expressed heretofore

1.    The Insurance Wavers and Releases apply equally to all items of the borrowed equipment for the duration of activities conducted by the Capital Land Lacrosse & Field Hockey Club, Inc. 

2.    We, the undersigned, will comply with the stated terms for use of such equipment.  Therefore, we are making the requisite rental fee plus deposit, each by separate check.  The escrow fee covers the replacement cost for each item of equipment borrowed, as outlined in the cost list on the reverse side of this form.  We understand the escrow check shall be held until said equipment is returned to the Capital Land lacrosse & Field Hockey Club, Inc. in usable condition, less normal wear and tear.  At that time, the escrow check shall be given back to me, unless one or more of the following conditions applies:

We agree to pay for any borrowed equipment that is not returned or, said equipment is damaged to the point that the same is no longer in usable condition.  The amount we shall pay will be the replacement cost as listed in the price index on the reverse side of this form.  Final determination of usable condition rests with the representative (s) of the Capital Land Lacrosse Club, Inc.

OR
We shall be allowed to purchase the equipment at the stated price listed on the reverse side of this form, as agreed to and arranged with the representative(s) of the Capital Land Lacrosse Club, Inc.
