Regardless of how minor an injury, the team's manager is responsible for filling out this form that details the injury and the incident that caused it. The manager is responsible for getting this to the League Safety Officer within 48 hours of the time of the incident. Beginning in 2006, any insurance claims that are filed that have not had the injury tracking form filled out within the 48 hour time limit will be reported to the Administrative Board and the Manager Selection Board for further action.
