Volunteer Screening Forms

Volunteer Application - This is a two page, letter size form.
Police Information Check - This is a two page, legal size form. It MUST be printed on 8.5" x 14" legal size paper (please do not reduce to letter size) as a two sided document, meaning page 2 must be printed on the back of page 1, and not on separate sheets of paper. Please do not sign the Police Information Check Form prior to submitting as it must be witnessed.
Identification - Please photocopy two pieces of acceptable identification on the same page and bring with you. The two pieces of I.D. may be (a) one piece of government issued picture I.D. such as Provincial Driver’s License, Alberta Identification Card, Passport, Citizenship Card, Native Status Card, Firearms P.A.L or P.O.L. Card, or Military I.D. Card PLUS (b) one piece of other government issued I.D. such as Birth Certificate, Social Insurance Number Card, Alberta Health Care Card, Immigration Documents, Marriage Certificate, or Permanent Residency Card.
Forms must be submitted in person to a member of our screening committee. Sessions will be scheduled for March and April for managers and coaches
