All three forms required for registration can be completed electronically and printed (note: you cannot save them, so make sure your printer is connected).

Aside from the three forms below, you will need three proofs of residency (a driver's license or state-issued ID, plus utility bills, voter registration card, homestead exemption card, mortgage statement or other bills that prove your address) and your child's original birth certificate.

The fee is $100, and $85 for each additional child, payable by cash or check.

2013 Player Registration form

The required medical release form contains information on your child's health, medical conditions, insurance company and pediatrician, along with emergency contacts.

Parents or guardians must acknowledge that they have read the handbook below (#4). It covers ballpark behavior and league rules.

PARENT CODE OF CONDUCT FORM

The Parent Handbook is required reading and outlines rules for each level of baseball and softball. Please make sure you know the guidelines so 2013 will be another enjoyable season for Bonita Springs Little League.
