2010 Manager Form contains both a Manager Application and Volunteer/CORI Form. TBoth forms must be completed and returned together. A copy of your Drivers License is also required. Applicaiton are due the first week of September. The "Manager Code of Ethics" should be signed and returned to your Player Agent.
