All Star Medical Release forms and other necessary documents.

The original Birth Certificate issued by the county not the hospital (No footprints)

A copy of parent’s WDL with current address
- A copy of parent’s phone bill or utility bill with current address

A copy of homeowners or automobile insurance with current address

Without these items and completed forms your child will not be eligible to play on an All-Star team. Birth Certificates will be returned at the end of the All-Star Season. Thank you in advance for your cooperation.

Bats with the special exception include the following:
The following bats do not cary the BPF 1.15 rating on them; however they are permitted to be used during play as per LL Int. If your player has one of the mentioned bats, please print the link page and keep it with you in your dugout as proof for umpire pre-game equipment inspection.
