Knights Sports Association Forms
All players must complete the Application form and return to your coach or mail it to the address on the form with the designated fee.
The Sponsorship form is to gather sponsors to off set your fees and travel expenses. All collections must be returned to the Knights Sports Association in order for the Sponsor to get credit with the IRS.
