Babe Ruth 14 Year Old State Tournament (Quincy, MA): Required Team Documents

REQUIRED TOURNAMENT DOCUMENTS

Babe Ruth League requires the following documents for each team before they are eligible to participate in any sanctioned Babe Ruth post-season tournament.

Tournament documents are to be placed in a three ring binder in the order listed below.  These documents are to be placed in clear plastic top loading paper savers before being placed in the binder.

1. Babe Ruth League, Inc. Letter of Eligibility Form
2. Any letter of waiver received by the League
3. Proof of league's accident insurance policy
4. Proof of league's liability insurance policy
5. Tournament roster certified and signed by league president
6. Birth documents - original birth certificate or Babe Ruth ID cards for each player.  Place in binder as same order as tournament roster.
7. Consent for Treatment forms (four copies per player).  Place in binder as same order as tournament roster.
8. Team photograph with player identification.  Identification must be on a separate sheet, not on the back of the photograph.
