AYSO Region 58: Registration

Drop Policy

When your child signs up for soccer, the Region incurs registration costs. In addition, team formation and balancing requires that we be advised of player drops immediately. Therefore, the following refund policy has been established:

  • Any Player who withdraws from the program prior to JUNE 30th (and including June 30th) is entitled to a refund of the registration fees paid less a $30 non-refundable registration processing fee.
  • There will be no refunds made after June 30th.

Open Registration

Please read the following carefully.

There are two (2) steps to register your child for AYSO Region 58 Fall 2014 soccer.

Step One – The first step is to complete and print two forms (before coming to the park to pay and complete the registration process), and have Proof of Age available if necessary.

  1. FORM 1 - Volunteer Sheet Form – you will need one (1) copy of the completed AYSO Region 58 Volunteer Sheet form. The Volunteer Sheet form can be accessed and printed by clicking here -> 2014 Volunteer Sheet. (Note that this form can be filled in on your computer and printed.) AYSO Region 58 is an all volunteer organization and only works because members of our AYSO community volunteer to help make it possible. If we do not have sufficient volunteers to make it possible to have a “fun, fair, and safe” youth soccer program, some registered players may not get the chance to play soccer this Fall 2014.
  2. FORM 2 - Player Registration Form – you will need to have one (1) copy of the completed, eSigned, three page Player Registration Form. You only need to bring the first two pages of the three page Player Registration Form. The Player Registration Form can only be completed using www.eayso.org. Instructions and a sample of a completed, eSigned, two-page Player Registration Form can be accessed and printed by clicking on 2014 Player Registration Instructions and Sample. If you have any issues with eAYSO (including system access, printing problems or your child not showing up as a returning player), please contact eAYSO Help Desk at 866-588-2976 (Mon thru Fri from 8 to 5) AND email stevenmeisberger@ayso.org, christopherkaufman@ayso.org, and ayso58registrar@gmail.com with the subject of "EAYSO Printing Problems" to ensure a quick reply to your issue. It is important the information on the Player Registration Form is accurate, especially the email address contact information. All routine contact and communications will be done using email.
  3. Proof of Age – you will need to bring Proof of Age if your child did not play AYSO Region 58 soccer in the Fall 2013 season. Proof of Age is normally a document issued by a government agency or hospital, for example, a Certificate of Birth or Passport.

Step Two – The second step is to bring the two forms listed in Step One, plus the Proof of Age if necessary, to the Van Nuys Sherman Oaks park (14326 Hartsook Street, Sherman Oaks, CA 91423) during a scheduled Player Registration event and submit payment, either check or cash (exact amount, no change will be provided) to complete the Player Registration process.

  1. No blank forms will be available at the park. You need to bring the completed forms with you to the park to complete registration. If you come to the park without the forms completed as instructed in Step One, player registration will be delayed.
  2. Submit Forms and Payment – you will need to bring either a check or cash to make payment. If you use cash, please bring the exact amount, no change will be available. FEES:

Early Bird registration fees (applicable ONLY to the December 7, 2013 registration date):

  • First Child $130
  • Second Child $115
  • Third and additional children $100

Registration fees for May 3, 2014 registration are as follows:

  • First Child $150
  • Second Child $135
  • Third and additional children $120

The sibling discounts noted above are ONLY applicable for children of the same family registering on the same day.

Solving Some Common eAYSO Printing Problems

1. I click the Print Button, but nothing happens – disable the Pop-up Blockers in your browser (see detailed instructions at https://www.eayso.org/eAYSOPopUpBlocker.aspx).

2. I click the Print Button and I can see the form on the screen, but the printed paper has none of my child’s information on it; it just prints an empty form with a bunch of boxes – you need to download and install the latest version of Adobe Reader:

a. Go to www.adobe.com.
b. Click on Download – Adobe Reader.
c. Choose the version of Adobe Reader that is correct for your computer and download it.
d. Install the Adobe Reader software and choose to make it your default program for reading PDF files.
e. Trying printing the form one more time (you might need to close the browser and re-open it).

3. I click the Print Button, and the form prints, but the left side of the form is cut off (including the first 2 or 3 characters of each word on the left side of the form) – the printer option “Fit to Page” needs to be selected from the Printer Options screen that pops up after the Print Button is pressed.

4. I click the Print Button, and the form prints, but the font on the form is clearly not English; it looks like random characters – you need to download and install the latest version of Adobe Reader:

a. Go to www.adobe.com.
b. Click on Download – Adobe Reader.
c. Choose the version of Adobe Reader that is correct for your computer and download it.
d. Install the Adobe Reader software and choose to make it your default program for reading PDF files.
e. Trying printing the form one more time (you might need to close the browser and re-open it).

5. I am using a Mac, and only the first page of the three pages prints when I click the Print Button – try Exporting to a PDF and then printing the PDF as follows:

a. Click on Export to the selected format
b. Select Acrobat (PDF) file
c. Click Export
d. Open the exported file (if it does not open automatically), and then print using Adobe Reader.
e. If any of the issues noted above in Problems #2, 3 or 4, occur, you need to download and install the latest version of Adobe Reader, per the instructions above and then try to print the exported file again.
