AYSO Region 58: Registration

Player Withdrawal / Drop and Refund Request Policy 

When you register your child for AYSO youth soccer the Region incurs registration costs.  In addition, the process of team formation and balancing of teams requires that the Region be advised of any player dithdrawals or drops immediately.  Therefore, the following refund policy has been established:

  •  When the Region is notified, as provided below, of a player's decision to withdraw / drop from the program on or before June 30, 2014, a refund will be made of the appropriate registration fee paid less a $30 non-refundable registration processing fee. 
  • If the Region is notified of a player's decision to withdraw / drop after June 30, 2014, no refund will be made.

Waitlist Process

Open player registration for the Fall 2014 Season is now closed.  Region 58 will ONLY be accepting player registrations off the 2014 Waitlist as described below.  We encourage parents to place their children on the waitlist as soon as possible, however, invitations to register are based upon the needs of the Region, including, but not limited to, volunteer requirements, team balancing requirements, and to fill openings on teams.

The waitlist process is as follows:

  1. Click on the link above to access the Waitlist Form.  Complete the Waitlist Form for each child you want placed on the Waitlist.  Only one child at a time can be added to Waitlist using the form, therefore, if you have more than one child that you want to put on the Waitlist you will need to complete the Waitlist Form once for each child.
  2. After you complete the Waitlist Form and submit it, a division manager will contact you when we have information about the number of open spots that we have for your child's age group.  This process can take between 2 and 4 weeks depending on the Region's requirements.
  3. If your child is 8 years old or older (as of 7/31/2014) and did NOT play at Region 58 last year, you will be asked to bring your child to a rating session.  These sessions are not tryouts, they are required so that we are better able to balance teams.  Participating in a rating session is not a guarantee that your child will be placed on a team, however, your child (8 years old and older) will not be placed on a team without attending a rating session.
  4. If there is an opening for your child, the division manager will let you know when you can come to the park to register your child during a Player Registration event.  Once again, this item 4 is by invitation only.  After you are notified that your child can register, please follow the steps detailed below in the Registration Processs section.


Registration Process

Please read the following carefully.  There are two (2) steps to registering your child for AYSO Region 58 Fall 2014 youth soccer.

Step One   The first step is to complete and print two forms before coming to the park to pay and complete the registration process, and if necessary, have Proof of Age for your child. 

  1. Form 1 -- Volunteer Sheet Form    You will need one (1) copy of the completed AYSO Region 58 Volunteer Sheet form.  The Volunteer Sheet form can be accessed, completed, and printed by clicking here 2014 Volunteer Sheet.  (Note: The Volunteer Sheet form can be completed using your computer and then printed.  As an alternative, you can print the blank form and then complete it manually.)    AYSO Region 58 is an all-volunteer organization and only works because members of our AYSO community volunteer to help make it possible.  If we do not have a sufficient number of volunteers to make it possible to have a "fun, fair, safe, and positive" youth soccer program, some registered players may not get the chance to play soccer this Fall 2014.
  2. Form 2 -- Player Registration Form    You will need to have one (1) copy of the first two pages of the completed, eSigned, three page eAYSO system generated Player Registration form.  The Player Registration Form can only be completed using the eAYSO system; access to eAYSO is at www.eayso.org.  Instruction on how to complete the eAYSO Player Registration Form and a sample of the first two pages of a completed, eSigned, Player Registration form can be accessed and printed by clicking on 2014 Player Reigstration Form Instructions and SAMPLE.   If you have any problems with eAYSO (including system access, printing, or, if applicable, your child does not show up in eAYSO as a returning player) please contact the eAYSO Help Desk at 866-588-2976, Monday through Friday, 8:00am to 5:00pm.    To help expedite a reply to your problem, please also email ayso58registrar@gmail.com with the subject of "eAYSO Printing Problems" and provide a brief description of the problem you are having.  It is important that the information of the Player Registration Form is accurate and complete, especially the email address contact information.  All routine contact and communications with player families will be done using the email addresses found on the Player Registration Form.
  3. Proof of Age    You will need to bring Proof of Age if your child did not play AYSO Region 58 soccer in the Fall 2013 season.  Proof of Age is normally a document issued by a government agency or hospital, for example, a Certificate of Birth or a Passport. 

Step Two   The second step is to bring the two forms listed in Step One, plus the Proof of Age if necessary, to the Van Nuys Sherman Oaks park (14326 Hartsook Street, Sherman Oaks, CA 91423) during a scheduled Player Registration event and submit payment to complete the Player Registration process.   Payment is made by check or cash.  If cash, please bring the exact amount; no change will be provided.

  1.  No blank forms will be available at the park.  You need to bring the completed forms with you to the park to complete player registration.  If you come to the park without the completed forms as instructed in Step One, player registration will be delayed.
  2. Submit Completed Forms and Payment   You will need to bring either a check or cash to make payment.  If you use cash, please bring the exact amount, no change will be available.  Checks should be made out to AYSO Region 58 and have the name of the child on the memo line.

Fees for Player Registration

The player registration fee for player from the Waitlist is $150 per child.  There are no sibling discounts for children registered from the Waitlist.

Solving Some Common eAYSO Printing Problems

1. I click the Print Button, but nothing happens – disable the Pop-up Blockers in your browser (see detailed instructions at https://www.eayso.org/eAYSOPopUpBlocker.aspx).

2. I click the Print Button and I can see the form on the screen, but the printed paper has none of my child’s information on it; it just prints an empty form with a bunch of boxes – you need to download and install the latest version of Adobe Reader:

a. Go to www.adobe.com.
b. Click on Download – Adobe Reader.
c. Choose the version of Adobe Reader that is correct for your computer and download it.
d. Install the Adobe Reader software and choose to make it your default program for reading PDF files.
e. Trying printing the form one more time (you might need to close the browser and re-open it).

3. I click the Print Button, and the form prints, but the left side of the form is cut off (including the first 2 or 3 characters of each word on the left side of the form) – the printer option “Fit to Page” needs to be selected from the Printer Options screen that pops up after the Print Button is pressed.

4. I click the Print Button, and the form prints, but the font on the form is clearly not English; it looks like random characters – you need to download and install the latest version of Adobe Reader:

a. Go to www.adobe.com.
b. Click on Download – Adobe Reader.
c. Choose the version of Adobe Reader that is correct for your computer and download it.
d. Install the Adobe Reader software and choose to make it your default program for reading PDF files.
e. Trying printing the form one more time (you might need to close the browser and re-open it).

5. I am using a Mac, and only the first page of the three pages prints when I click the Print Button – try Exporting to a PDF and then printing the PDF as follows:

a. Click on Export to the selected format
b. Select Acrobat (PDF) file
c. Click Export
d. Open the exported file (if it does not open automatically), and then print using Adobe Reader.
e. If any of the issues noted above in Problems #2, 3 or 4, occur, you need to download and install the latest version of Adobe Reader, per the instructions above and then try to print the exported file again.
