AYSO Volunteer Form

Registration is currently underway now through July 16. Don't wait and don't be late. By the time school is back in session in mid-August, our soccer teams will have already been formed and most likely filled.

We fill teams by post marked date from registration forms. The last open spots on teams will be filled by the date in which their registrations are received. So don't wait, or you might be too late. Form is in Microsoft Word format.

A $10 late fee must be included in your payment if your registration is received/postmarked after 7/16/08.

Please make sure we have an updated email address on your registration forms.
