ASUSC Scottsdale Soccer

Rec Open League (Second Season) team information
This information is needed to register Recreational teams that will be playing State Open League, which is also referred to as Second Season. Participating teams have identified themselves to treasurer@asusc.com, have paid the required team registration fee, and sent a roster (player name and date of birth) to registrar@asusc.com. The paperwork step includes completing a Disclosure form for each adult who will be working with the team (coach, assistant coach, team parent, lead chauffeur, etc.) and returning that to the Club registrar as shown below. The balance of the forms are to be completed as indicated and kept in a binder that accompanies the team to games. If you have any questions, please send Email to registrar@asusc.com

Second Season / after the paper work. . . .
Once the team has completed the above, player ID cards (and coach / assistant coach / team parent ID cards) will be printed on AYSA ID card stock. To complete the ID cards, the coach or team parent will need to collect a picture of each player and adult that can be trimmed to an approximate one inch by one inch "face shot." This picture will then be positioned on the ID card where indicated and the entire ID card will be laminated. The lamination can be done at most office products stores such as Kinko's, etc, for a nominal cost. The resulting ID cards should be single hole punched, put on a key chain-like ring, and taken to every game. These ID cards will then be used during team check in with referees at the beginning of each Open League game.
