URGENT, PLEASE DO IMMEDIATLY.  All parents need to fill out the survey.  It gives me a look into the future so to speak.  Dont worry it is confidential with the exception of the last few questions about your specific child.  Please answer honestly.  We want to improve and if you sugar coat it or give it all a GO how do we help you child or others in the future.  We have thick skin so please answer the questions and give us as much feedback as possible.  Please express any other items you feel you would like at the end.  I am looking for the negatives so we can fix them.  That said we try hard at this and would like to here the positive also so we know to keep going and feel good about what we do.
