Please use the following form for refund requests.

Please keep in mind the following:
       1. The $50 registration fee is refundable ONLY if you move out of the district or cannot play due to weight limits
       2. The $50 uniform fee is refundable ONLY if:
             --your check has cleared the bank,
             --your uniform and equipment have been returned,
             --your fundraising money has been returned, and
             --you have not taken part in any games, including Preview.
Refund Request slips should be turned in to your parent rep, or directly to Peggy Busacco at 436 Heathgate Rd, Cincinnati, Ohio 45255. Refund checks are written once a month, so it may take a while to receive your refund.

Be sure to fill out the form completely, especially re: how the check should be made out. Thanks.
