Anchorage Military Community Little League: Required All-Star Paperwork

During the All-Star try-outs/Registration, players must provide the following paperwork & other requirements.

1.  Original Birth Certificate:  Any player wanting to participate in All-Stars must provide their original birth certificate (will be returned after the District #1 Little League All-Star Affidavit Meeting) and a copy.  Note: California Birth Certificates require additional information - Please ask.

2.  All-Star Registration Form

3.  All-Star Medical Release Form

4.  All-Star Player/Parent Code of Conduct Form: Signed by player & parents.

5.  Players Must have played in 60% of the regular season games. Note: Junior Players playing High School Baseball & Softball games count towards Little League (require waiver letter).

6.  Cost: Baseball & Softball, $50.00 per player. Payment must be turn-in at tryouts. Payment must either be paid with money order or check made out to the Anchorage Military Community Little League. We will accept cash as well.  Note: Any player not making All-Stars, money will be returned.

 7.   JBER/AMC Boundary Paperwork: This only applies for any player who is playing for AMC who lives off-base. All active duty military or DOD civilian working on base are waivered by Little League and can play on base or for the league where they live at. Note: If you live off-base and are playing All-Stars, we will need proof of parent’s active duty status or DOD Employee status (example: copy of orders, ID card, CAC Card-Civilians or LES).

 8.  New Players Residency Waiver: This only applies to any player that moved to JBER (Elmendorf-Richardson) District after 1 February 2012.  We have to do a Little League waiver allowing the player to play during the season.  We will need a copy of your PCS orders showing you moved to JBER after 1 February to attach with the waiver.

9.  Residency Paperwork: Players must provide residency (boundary) paperwork as well.  The paperwork must show their current address and proof must be dated between 1 February 2011 & 1 February 2012.  Players will need one form of paperwork from at least three different categories - see below for categories:

    A.  Driver's License

    B.  Voter's Registration

    C.  School Records

    D.  Welfare / Child Records

    E.  Federal Records

    F.  State Records (i.e. fishing or hunting license)

    G.  Local (Municipal) Records

    H.  Support Payment Records

    I.  Homeowner or Tenant Records

    J.  Utility Bill (i.e. gas, electric, phone, heating, etc.) - only one works for this category

    K.  Financial Records (i.e. loan, credit, investments, etc.)

    L.  Insurance Documents

    M.  Medical Records

    N.  Military Records

    O.  Internet, Cable or Satellite Records

     P.  Vehicle Records

    Q.  Employment Records

    R.  *Combined Team Record Form - Only if combining w/another league

See attached handouts for the All-Star Player/Parent Code of Conduct Form, All-Star Medical Release Form and AMC Little League Interest & Availability Application form. All these forms, plus ORIGINAL birth certificate, payment and other required documents must be completed and turned-in at try-outs (note: please read AMC Little League Interest & Availability Application form for additional paperwork). Athletes have to have lived in the current residency prior to 1 February 2012. If not, and we have not done a waiver already, the player will be not eligible to participate in All-Stars.

We will need the original birth certificate and a copy. The original birth certificate will be returned after the District #1 affidavit meeting (late June).  All players must be in uniform, we will be taking the All-Star Team photos after the try-outs.
