Volunteer Application

All volunteers must complete this form and submit it along with a photocopy of a valid state issued ID for a background check prior to assuming volunteer activities. each volunteer should complete the Volunteer Opportunity form to assist our league. New in 2010, volunteers will be required to wear a League-issued photo ID Badge.

Registration: Concussion Law

Handouts related to the new Washington State Concussion Law. All players and parents must sign the Concussion Information form prior to participation in our program. More information on this law can be found in the "Safety" section of this website, including links to other educational websites.

Required for all players league age 5 - 12. One form per player.

Registration: Player Placement for Minor Divisions

Parent-Player Request for Team Placement: Optional form for players league age 5 - 11.
Player Options for Minor Division:Required for returning league age 6 year olds; all new & returning league age 8 year olds; and Baseball players new & returning league age 9 year olds.
12 Year Old Option: For league age 12 year olds entering the draft process that do not want to play in the Major Division.
