Game Report Forms
Game reports must be completed for every game to which you are assigned as the center referee. Even if you arrive at the field and one or both teams do not show up for the game, you must still file a game report. In addition to filing your report with the league, you must also forward a copy to EPSARC in the event that you issue a sendoff (red card) or if a player suffers serious injury and is unable to return to the match. All game reports should be placed into the mail within 48 hours after game time.
There are two versions of the game report available for you to download. Both are in Microsoft Word format. Either click on the "Handout: Game Report Forms" link on the middle of this page, or go to the "Downloads" folder on the left-hand side of this screen.
The first version has dropdown boxes - the perfect solution for those of us whose handwriting looks like chicken scratch. After you complete the form on your computer screen, print it out and mail it in. This version also includes the Supplementary Game Report form as page 2 of the same download. Note that if you try to print this version as a blank form, it will automatically use the first entry of each dropdown box (ie: conduct of officials, players and spectators will always be "excellent"; marking of field will always be "good", etc...)
If you prefer to fill out the game report by hand, use the second version. You can print out the blank form on your printer and make as many additional blank copies as you like before you fill in the information by hand. If you use this version and you also need to file a Supplementary Game Report form (misconduct, serious injuries, failure of a team to appear for the game, etc...), you will need to download that form separately.

DELCO League Game Reports
Within 48 hours of the game, go online to www.delcosoccer.org and complete the report directly on the DELCO League website. If you issued any cautions (yellow card) or sendoffs (red card) or had any other reportable incidents, fill out an incident report.
