  • X-Calibur Forms - Recruit, Try-out, Code of Conduct, Waiver Forms. The Try-out form is for anyone interested in attending team trials. The remaining forms are required for members of the travel teams.
  • Academy Information - Registration information for the Academy program.
  • X-Calibur Letters - You MUST be a parent or player AND have an eteamz log-in to access this section of "Forms & Handouts". To get a log-in, you must send a request which includes your email address. The club will try to activate your account within 24 hours. From time to time the club will post important information here for parents and players. When you receive an email from the club asking you to read something posted here, please do so in a timely manner.
  • Clinic Registration Forms - Download the form for the clinic you are interested in. Please register as early as possible, as spaces are limited for most of these clinics.
  • Roster Forms & Rules - Coaches should print out appropriate the ROSTER form(s) for those tournaments requiring one, and bring them to bring to a practice BEFORE and event- have a parent fill them out for you (they love to help out....). Please try to complete all information before arriving at a tournament to register your squad. RULES are for anyone interested in the game to download.
  • USFHA Forms - USFHA Waiver, Membership, Futures Attendance Forms. Please be sure your membership is VALID through the end of any USFHA event. Keep your proof of membership with you for all tournaments.
