*League Shirt, Hat and Trophy included in registration fee.
*A $5.00 fee shall be added per family for all out-of-county residents.
*A $5.00 late fee shall be added to all registrations received after 2/4/08.
*All players will be responsible for purchase of pants, belts and socks.

REQUIRED AT TIME OF REGISTRATION....
Three proofs of residency and a copy of each players birth certificate.
