Please Note: There will be one football team per division. Sign-ups are taken on a first-come, first-serve basis.

1. FOOTBALL PLAYER (must weigh in & size)
2. PARENT / GUARDIAN
3. Two (2) Xerox Copies of Certified BIRTH CERTIFICATE
4. HEAD SHOT PHOTO (Wallet-sized - NO hats!)
5. REGISTRATION FEE: Checks, Credit Cards or Cash

FOOTBALL: Includes everything player needs (except cup and cleats). Player keeps Jersey(s), Trophy, Pictures, and Spirit Pack.
CHEER: Includes uniform (spirit pack, shell, skirt, breifs, warm ups, socks, shoes , bow) Plus 1 day Palomar Conference Spirit Camp, Trophy, pictures, and Palomar Conference Cheer Competition.
