Registration and fittings will be held on the first Sunday you attend at Heritage Elementary School from 1:00PM and practice will follow from 1-3:00PM (see locations link on left for directions) No Tryout required.

 

Practice

Club Fees

Ø One 2 hour practice a week

− Nov 18th  – May 19th

−Sunday afternoons (except holiday weekends)

−Time: 1pm - 3pm

  Payment Due at Registration $275  

  Payment of                             $125
  (Due Jan.)

Total Club Fees                     $400

Ø Fees include Practice T,
   spandex and knee pads

USAV Membership                      $25

Total Cost to play                  $425

·         2013 AZ Region USA Volleyball Membership Card (completed online)

**PRINT out and bring to tryouts your daughter’s USA Volleyball membership card showing renewal through 2012 (we will keep copy)**

·         2013 AZ Region USA Volleyball Medical Release Form (Printed from online registration or see Format & Handouts link to left)

·         2013 White Tank Mountain Tryout Registration Form (See Forms & Handouts link to left)

·         Arizona Region Concussion Form (See Forms & Handouts link to left)
