2011-12 Ticket to Play time lines
*Check web site for changes regularly and please do not mail physicals*
Spring Sports Practice begins Saturday, February 18, 2012.
* Deadline for Paperwork Drop-Off: Completed 7-page physical and 2-athletic handbook signature pages must be turned in at the Westview Athletics Office before Wednesday, February 1, 2012.
Ticket to Play Pick-Up: Monday, February 13th through Friday, February 17, 2012. Times: Before School, Lunch, and After School to 3:30 p.m. (No School February 20th to 24th)
To get your “TICKET-TO-PLAY”, fill out and print all 9 pages listed below. All Ticket-to-Play forms must be updated each school year.
Physicals are good for 1 year from date of examination and cannot expire during the sports season. Only PUSD Athletic Screening History & Physical Exam forms are accepted.
Bring ALL signed forms to the Athletic Office to receive your ticket:
- Physical & Medical Information Release Form (7 pages)
- Athletic Participation form (2 pages with signatures): a. Athletic Handbook, Hazing and Ethics Signature Form (1 page) b. Residency & Eligibility Verification Form (1 page)

Lacrosse Tryouts & Practices Begin February 18-26

BOWLING FUNDRAISER!!
When: Friday, Jan. 27th Professional Growth Day-so kids are off!
Time: 12:00-2:00 pm (Check in at 11:30am)
Where: Mira Mesa Bowl (8210 Mira Mesa Blvd. CA 92126)
Cost: $20 per bowler (covers 2 hrs of bowling, shoe rental, prize giveaways , and raffle ticket)
