The following items are required at registration:
- Original or certified copy of Birth Certificate and photocopy
- Signed Code of Conduct forms (Parent and Player)
- Completed Medical Release form
- 3 forms of proof of residency and photocopies of each - see Forms section
- If you're planning on volunteering as a Manager, Coach, Umpire, or any other position during the 2013 calendar year, please bring a completed 2013 Volunteer Form (forms section) and a copy of a photo ID.
