Wednesday, June 4th  4:30pm - 6:30pm

Honowai Field

Tee Ball ( ages 5-6)  $105.00 

Pee-Wee (ages 7-8) $110.00

($25 of the registration fee will go to the team to use for trophies, end-of-season potluck)

The following items are required at registration:

• Completed Registration form
• Completed Medical Release form
• Birth Certificate (original or certified copy)
• 3 forms of proof of residency (ex. auto registration or insurance, homeowners insurance, rental agreement, one utility bill, parent ID, bank statement, etc.)
