Western New York High School Club Hockey League: News: Required Paper Work For 2009-2010
Required Paper Work For 2009-2010The following is the paperwork required for ALL Teams.
1. A letter from the School on school letterhead with raised school seal, with the players names and grades signed by the AD or school Principal. OR, a copy of the players school ID or report card.
2. Completed NYSCSH Roster form with AAU Registration #.
3. A copy of each players birth certificate.
4. Completed NYSCSH Waiver of Liability form for each player and coach.
5. Completed NYSCSH Consent to Treat form for each player and coach.
6. Completed NYSCSH Code of Conduct forms for each player and coach.
7. Up to date copies of the coaches Coaching Cards (front and back).
