Include " 2 COPIES" of your son or daughter's Birth Certificate (ORIGINAL WILL NEED TO BE TURNED IN - JULY2012), Physical Form (Current Year Only - dated 2012), 2 Copies of Report Card and 2 copies of a Utility Bill for address verification.

2012 Contract Form

All "Adult" Volunteers must be LIVE-SCANNED and "Cleared" prior to season start to work with any children in this organization.

Volunteers must do this prior to June 1st, 2012. Turn-around time for clearance takes anywhere from 2-10 wks.
