Include " 2 COPIES" of your son or daughter's Birth Certificate, Physical Form dated 2011, 2 Copies of Report Card and 2 copies of a Utility Bill for address verification.

All "Adult" Volunteers must be LIVE-SCANNED prior to season start. Live Scan Forms must be requested via email, at:

Volunteers must be "CLEARED" (Live-scan) prior to working with any children in this organization.

Volunteers need to LIVE SCAN prior to June 1st, 2010. Turn-around time for clearance takes anywhere from 2-10 wks.
