To simplify registration for the 2008 season the forms that are necessary are posted below. Please download them, fill them out and bring them to registration.
• Participant Information – Each Player needs a Participant Information form. These are preprinted for returning players and that data needs to be verified by the parents at registration. New players need to fill this form out.
• Physician’s Certification Form – Each Player must be examined by a Physician and certified that they are physically able to play football or be a cheerleader. This form should be completed by your doctor, Signed and Stamped, after the child is examined and turned in to your child’s coach before the start of the season. Jersey Shore requires that the Pop Warner Physician's Certification be signed and stamped by your physician.
• Proof of Address -- Each Family needs to complete this form, listing all children in the household that are registering for TRLI football or cheer this year.
• 2008 Policies and Procedures --These are the policies and procedures used to run the organization. The document is under review and will be available soon.
