Forms to Bring 1st Day of Tryouts

In addition to the forms below, please bring the following: $25 tryout fee, SRVA online membership form, $60 check payable to Tri-Cities Extreme for SRVA membership fee, SRVA medical release form - notarized, copy of insurance card and birth certificate. IMPORTANT - - All prior insurance cards and birth certificates received by the Club are destroyed at the end of each season. Also, the $25 and $60 fees need to be remitted in different checks please.
