This is a single page document, revised as of 01/03. Needs to be completed for each Player and Coach and kept on record by Team Manager for Troy-Albany Youth Hockey. Required documentation for NY State Sectionals and State Tournaments.

USA HOCKEY CONSENT TO TREAT FORM

This is a two page document, needs to be printed front and back on a single sheet of paper. Needs to be completed for each Player and Coach every season and Travel with team. Required documentation for NY State Sectionals and State Tournaments.

USA HOCKEY PARTICIPANT CODE OF CONDUCT

This is a single page document, needs to be completed by each Player every season and kept on record by Team Manager. Required documentation for NY State Sectionals and State
Tournaments.

COACHES SCREENING FORM

This document, needs to be completed by each coach every season and provided to the Association President. Required by The New York State Amateur Hockey Association.

NYSAHA RELEASE FORM

This a single page document, is required when registering with Troy-Albany Youth having played elsewhere the previous season. Needs to be completed and signed by your current association president before Trying Out for a TAYHA Team. Required by New York State Amateur Hockey and The Troy-Albany Youth Hockey Associations.

This is a single page document, needs to be completed for each player registering with Troy-Albany Youth Hockey.

PLAYER EVALUATION FORM

This a single page document, should be completed by each House League Coach to evaluate each of his players. Team trophies will be released to coaches only after forms are returned. Thank You Coaches!
