This is a two-page document, needs to be printed front and back on one sheet of paper. Should be completed for each player on your team, every season and needs to travel with the team. Tournament Bound Teams will need to provide this form at NYS Sectionals, State Finals and National Tournaments. (Printing Hint: Print side one only, flip that page, reinsert into your printer and print side two.)

Consent to Treat Form

This form should be completed by each coach on your team and turned into TAYHA President, at the beginning of each season.

Screening Information Form

A single page document, should be completed by each player on your team each season. Tournament Bound Teams will need to provide this form at NYS Sectionals, State Finals and National Tournaments.

Participant Code of Conduct

This is a one page document, should be completed by each House League Coach to evaluate each of his players. Forms are to be returned to the level Coordinator no later than 3/1/02. Team trophies will be released to coaches, only upon the return of this form. Thank You Coaches!
