Late Registation for 2009 Fall Soccer Season

The Swedesboro-Woolwich Soccer Association is now accepting late registrations for the Fall 2009 Soccer Season for the Micro and PeeWee Programs.  Email the board for information if you are interested in registering your child for Fall Soccer.  Spots will be filled on a first come, first served basis.

You may register your player by mailing the following completed forms and information to the SWSA Board:

1.  2009 SWSA Registration Form
2. 2009 Medical Form (note: Medical Form MUST be notarized)
3. 2009 Code of Conduct Form
4. Copy of child's Birth Certificate (needed for ALL players - even those that have played before)
5. Travel players - wallet size picture
6. Check for Registration Fees - payable to SWSA 

Registration Fees:

• Travel (must be 8 by July 31, 2009): $125.00
• Travel Uniform (if needed): $70.00
• Youth Development Program (born between 8/1/97 - 9/30/01): $95.00
• Micro (born between 10/01/01 - 9/30/04): $75.00
• Pee Wee (born between 10/01/04 - 9/30/05): $35.00
