To register for the 2009 Fall Season you must complete the following three forms:

ALL Coaches and Team Parents must complete the 2009 Kid Safe Disclosure Form.    ALL Traveling Coaches must complete the 2009 Coach Member Form. These forms must be filled out EVERY year.
