You will need to complete the registration forms located in the handout section and include a copy of a parent or guardian drivers license as well as a copy of the birth certificate for the child being registered. We CAN NOT accept the registration forms without the supporting documentation and funds at time of registration. There will be no "holding" of registration forms.

When filing out the forms, they must be complete. Please include your mailing address as well as physical address on the registration forms.

We can not accept forms just listing a P. O, Box, there MUST be a street address listed as well.
