Registration Items needed:
1) Original/Certified Birth Certificate - Please bring 2 copies.
2) 2x2 Photos - Needed for player registration & the books.
3) Sports Physical - Physical must have original doctor's signature and stamp. Your son/daughter may not participate in practice or games without this document.
4) Full Years 2011/2012 Report Cards. Must be turned in no later than July 30, 2012.
