2009 Registration Requirements:
All participants are required to be present for registration. Please bring with you the following documentation:
- Original Birth Certificate
- Current Year's Report Card
- Proof of Residency (Water, Gas or Electric Bill) of parent or legal guardian
- Pop Warner 2009 Official Physical Form (completed as of January 1, 2009 by a licensed physician)
