LOCATION: Senior Citizens Community Center, 515 East Dewey Street6:00 to 8:00 p.m.
COST: $50 first child, $35 each additional sibling
Detailed information and signup forms can be downloaded here. A separate Player/Parent Contract AND Parent Code of Ethics must be completed and signed for EACH child. A Waiver and Release form must also be completed for any player that is playing up an age classification (i.e., 6&under eligible, playing 8&under)
Completed forms may be brought to signups or mailed along with your check or money order to the address shown on the signup form.
