Need a release? You must be in good standing to receive a release. Print NYSAHA Release form, |

fill out Part I, and mail it with a self addressed, stamped envelope to the address on the form. All |
| |
|
requests will be returned within 10 days from receipt. Make sure to allow sufficient time. |
