If possible download and print all the documents below.All of these items have vital information required to be able to complete your childs registration. And having them filled out will reduce your sign-up time. These forms will be available at the sign-up locations if you have any problems.

Please check the following:
1. The little league zone map, you must reside inside of the dark green zone in order to be eligible to play for our league in the 2008 spring season.
2. The proof of residence, you must provide
   3 original docs from the list to prove residency.
3. The proof of age requirements, Only the docs listed will be acceptable proofs of age.
4. Codes of conduct, must be signed by each parent and each player signing up.
5. Volunteer App. if you are wanting to manage,coach,team mom, or help your childs team or the league in anyway which puts you in contact with the leagues players you must fill out a App and have a back ground check done. A copy of your photo ID will also be needed along with $5 to cover the cost of the back ground check.

Sponsorships & Donations by local businesses give Sunrise Mountain Little League the ability to operate and supply our communities youth the chance to learn and play the great game of baseball. They also keep the cost to the parents at a reasonable price. Without these sponsors the league would have no choice but to raise the cost of signing up and that would reduce the amount of children who could afford to participate. So help us by printing out these docs and giving them to all the potential sponsors in our community. Make sure they are filled out completely and given to our league treasurer in a timely manner.
