REQUIREMENTS FOR REGISTRATION:

**Child and Parent or Legal Guardian MUST be present for registration**



All participants must have the following:

Original Birth Certificate (plus 3 copies)

Copy of FINAL report card for 2008-2009 school year

Current 1x1 photo

Two types - Proof of Residence (plus 2 copies); any two of the following

-SDG&E Bill

-Water Bill

-Renters Insurance or Agreement

-Report Card with Address

-Car Insurance or Registration

-DMV License or Identification

Half of Registration Fees

REGISTRATION FEES:

Football: Flag $165, Mitey Mite-Midget (tackle) $215

Cheer: $165 (this does not include the cost of attire.
