South Austin Lacrosse Association RAFFLE

Tickets can be purchased @ games and Wednesday practices:

Registration Requirements
Checklist for Registration: (Please turn in all forms ASAP)
- SALA Lacrosse Registration Form (download from site Handouts)
- SALA Permission and Waiver Form (download from site Handouts)
- SALA Player Medical Release Form (download from site Handouts)
- SALA Code of Conduct Form (download from site Handouts)
- SALA Physician Approval Forms (download from site Handouts)
Note: Players can use the same physicals from another sport if physical is from within the past year.
- US Lacrosse Membership (register via www.uslacrosse.org)
SALA Registration Cost: $165, boys & girls
If forms and payment are not turned in by this Saturday (unless notice has been given to the Coaches) players cannot particpate in practices.
