This form must be completely filled out for each child registering to play baseball for Quito Little League.

2010 Medical Release Form

Per league requirements, this form must be completed by every manager, coach, parent and volunteer.
This form must be submitted with photo identification: Your Drivers License is a preferred example.
This form must be completed NEW each calendar year, if you are volunteering for spring ball and again for fall ball, please check with the league president to determine if you will need to complete a new form for fall ball. Thank you!
