Use this manual form to register your child for Polar Little League's 2012 Season.

Refer to the 2012 Registration Fee Schedule to see what the cost is per player for each league / age group.

2012 Registration Fee Schedule

In case of emergency, if family physician cannot be reached, this form gives the authorization for your child to be treated by Certified Emergency Personnel. (i.e. EMT, First Responder, E.R. Physician)
