Medical Release Form
- A completed Medical Release form is required for every player before the first practice. Please print, complete and bring the form to In Person Registration.
PALL Mgr/Coach Evaluation
- PALL invites parents and players to use this form at the end of the season to provide feedback on your team's Manager and Coach(es). Please bring it to In Person Registration or mail it to the P.O. Box shown on our website's home page.
