Volunteer Application
In order to protect children and maintain a hostile environment for those persons who sexually abuse or exploit children in any way, the local league personnel listed below must probide a consent to a background check. Persuant to Regulations I (c) 8 and 9, local leagues are required to conduct annual background checks of all individuals required to submit a Little League Volunteer Application prior to the applicant assuming his/her duties for the current season. Any person with a conviction of a crime against or involving minors cannot participate in the local league.
The local league board of directors must ensure that all of the following personnel have annually completed and submitted, to the local league president, a Little League Volunteer Application, prior to the applicant assuming his/her duties:
1. Managers
2. Coaches
3. Board of Directors
4. Any other person, volunteer and/or hired workers, who provide regular service to the league and/or have repetitive access to, or contact with, players or teams.
Refusal to annually submit a fully completed Little League Volunteer Application must result in immediate dismissal of the individual from the local league.
The Little League Volunteer Application must be maintained by the president of the local league board of directors for all of the personnel names above. The president must maintain the applications, at a minimum, for the duration of the applicant's service to the league for that year. Failure to comply with this regulation may result in the suspension or revocation of tournament privileges and/or suspension or revocation of the local league's charter by action of the Charter or Tournamment Committee in Williamsport.

Medical Form for Players
Managers are required to collect a medical form for each player and keep them in the binder found in each equipment bag. Players are not allowed to practice or play in a game without the medical form. The form can be found at http://www.littleleague.org/Assets/forms_pubs/asap/Medical_Release_Form.pdf.
